Employees are required to report any workplace injuries and illnesses to their supervisors immediately. Similarly, it is imperative that all students promptly report any injuries occurring in labs, classrooms, or other instructional spaces to their faculty members. The supervisors and faculty members will then be responsible for completing the relevant injury reports. Timely reporting ensures proper injury care, appropriate documentation, and necessary actions can be taken.

It is the supervisor's responsibility to ensure all injuries, accidents, and near misses that occur to their employees while in the workplace and students while in the instructional space are reported and corrective actions are taken to prevent other injuries.

It's important for employees and students to report to their supervisor or faculty member any near-misses that had the potential to cause serious injury or harm in the workplace or instructional space. Near-miss reporting provides the supervisor with the opportunity to correct potential hazards before an injury occurs.
