Administrative Appeal
Students may submit an Administrative Appeal if the issue pertains to one or more of the acceptable situations outlined on the Administrative Appeals Procedure webpage.
Administrative Appeals must be filed on or before the deadlines listed below, immediately following the semester during which the situation occurred.
- Fall semester deadline - March 15
- Spring semester deadline - August 15
- Summer semester deadline - November 15
Application for Degree and Certificates
Prior to submitting an application for an Associate Degree, Certificate of Achievement, or Certificate of Completion; students should consult with an advisor to review progress toward completing requirements. Applications are to be submitted to the Registrar's Office the semester the program requirements are completed on or before the deadline:
- March 15 - Last day to apply for Spring semester degrees and certificates
- July 15 - Last day to apply for Summer semester degrees and certificates
- November 15 - Last day to apply for Fall semester degrees and certificates
CHANGE OF ADDRESS
You can update your address by going directly to Banner Self-Service. Just log into myLCC, select Banner Self-Service, click "Personal Information" and then select "Verify and Update Addresses and Phones". Follow the steps provided to update your address. If you have an address hold on your account, it will be released in approximately 15 minutes after submitting your updated address.
If you are unable to update your address in Banner Self-Service, then complete the Change of Address form and we will update your address. The processing time is 1-3 business days. Changing your address record to an in-district address does not make you automatically eligible for a lower tuition rate. You must submit a Student Declaration of Residency form with documentation to the Registrar's Office for a residency status review.
Change of Legal Name
You will also need to submit a legal document such as an enhanced driver's license, enhanced state id, marriage license, passport, or court document. Social security cards cannot be accepted as legal documentation.
Chosen Name Change Form
A new procedure now allows all LCC students to select the name they use that will be integrated into most systems at the college.
Enrollment Verification Request
Enrollment and degree certification provides confirmation of enrollment status (e.g., full-time, part-time), dates of attendance, and degree(s) conferred. Certification is used to confirm this information to insurance companies, lenders, employers, and other organizations. You can also get this information instantly in myLCC.
Registrar’s Office is not able to verify enrollment statuses until the 50% drop period for the full semester period has ended. Please allow 3-5 business days from this date for completion and response regarding your request. Drop period information for each semester is available in the Academic Calendars located on the Registrar’s Office webpage.
Applied but never Attended?
If you've never attended Lansing Community College and want to request proof of no academic history, you can skip the form and send the request via email to registrar@lcc.edu. The request must include a valid photo ID, along with the receiving email where the letter should be sent. Please allow 3-5 business days for requests to be processed.
High School Dual Enrollment Form
High school and home school students must submit forms every semester prior to registration. Please allow 7-10 business days for processing.
Honors Letter Replacement Form
Use this form to request a duplicate Dean's or President’s List letter for any semester after Spring 2010.
Identity Theft & Fraud Claim
Individuals who believe their identity has illegally been used to apply at LCC and receive financial aid should complete this form. To begin this form you will need to create a Dynamic Forms account; this does not require an LCC student account. Within the form you must submit one of the following: police report (may complete with LCC DPS), loan servicer claim, a claim confirmation with the Federal Trade Commission (FTC), or Department of Education (DOE). Please allow for 30 - 60 days for internal processing and final determination of claim with LCC.
Program AND CATALOG YEAR UPDATE REQUEST
Use this form to request a change of program and/or update your catalog year for the semester(s) listed below. Catalog year refers to the academic years of the program pathway. Select the catalog year closest to the courses you have already completed and in the area of study you wish to pursue. Please allow 7-10 business days for processing.
Note: Due to federal regulations, changing your program may impact your financial aid eligibility. For more information, please visit the Courses in Program of Study section of the Financial Aid Policies page or contact Financial Aid at 517-483-1200 (option 1)
Professional Licensure and Certification Disclosure: There may be a need for additional education requirements or special authorization from professional licensing or certification agencies located outside the state of Michigan. Please read through our Professional Licensure and Certification Disclosure for more information.
Release of Information (FERPA)
This release will go into effect on Monday, January 7th, 2019. All previously submitted paper versions of the form will no longer be valid. In accordance with The Family Rights and Privacy Act (FERPA) of 1974, LCC cannot discuss your academic and/or financial information with your parents, spouse, or guardian. By completing this dynamic form, you authorize LCC to discuss this information with your designee. Note: This form can also be used to add, remove, or change your designee(s) on your Release of Information. Please contact the Registrar's Office at 517-483-1200 if you have questions.
Request for Duplicate Acceptance Letter
Use this form to request a duplicate LCC Acceptance Letter.
Request to Prevent Disclosure of Directory Information
Prevents the college from disclosing directory information to anyone other than the student, their designee, or exceptions as defined by FERPA. Having a Prevent Disclosure of Directory information on file does not prevent staff from communicating with the student or their designee over the phone or other communication methods upon authentication of identity.
SOCIAL SECURITY NUMBER VERIFICATION/CORRECTION
Students can submit this form to update or verify their social security number with the college.
STUDENT DECLARATION OF RESIDENCY
Students who have been admitted as out-of-state or out-of-district may submit this form with supporting documents to get a lower tuition rate. Please allow 7-10 business days for processing.
STUDENT DROP FORM
Students can use this form in order to drop a course electronically in the event that they are unable to do so from their student account.

Use this form to order a replacement diploma or certificate, or to order a tassel or honor cord. These items can also be ordered using the Student Graduation store in Marketplace. There is a fee for each item.
International Student Forms
These forms are only for international students who are currently using an F1 visa.
Late Registration - Schedule Adjustments
Students are allowed to enroll in classes and change their schedule in Banner without faculty approval or staff assistance through the first week of class for 16-, 15- and 14-week courses. For 12-, 8- and 6-week courses, students have up to three days to enroll or change their schedule.
For those late enrollment or schedule adjustment requests after the first week of class for 16-, 15- and 14-week courses or after the first three days of 12-, 8- and 6-week courses, special circumstances may be considered after consultation with and approval from faculty and with final approval from the Academic Dean.
Students must complete the student section of the Drop & Add Form below and submit it via email to the respective department for processing. Please note that the department will coordinate with faculty and dean to obtain the required signatures for the student.
