After reading our guide, please complete a Transfer Success Form (TSF). Once you have submitted a TSF, we will carefully review your information and reach out to you directly to schedule an appointment. Appointments are offered in person, virtually, or over the phone.

To order you will need to have the following:
- Your Student ID or Social Security Number
- An email account
- A credit or debit card
