Benefits Intent Form

The Intent Form tells the Veteran Services Office that you would like to utilize your benefits for the upcoming semester. This means that the Intent Form must be completed each semester, AFTER you are registered for your courses. If you complete the Intent Form prior to being registered for courses, it will be rejected. Statement of Understanding

The Statement of Understanding provides policy and process information for LCC and the VA. This form is only completed ONCE when you are a new student. If you have any questions about items on the form, please contact our office.

Release of Information

The Release of Information allows a student to select someone who will be allowed to contact the Veteran Services Office on the student's behalf and discuss school and military benefit information. The student can select multiple people if they would like. If the student doesn't want to select anyone, they do not have to. If that is the case, then the only person our staff can speak with regarding course and benefit information is the student. This form is only completed ONCE, unless you would like to update the form. Letter of Eligibility Form

This form simply allows you to upload your VA Letter of Eligibility and submit it to our office so we can place it in your student file. We ask all students to provide a copy of the letter so our office knows exactly which benefit you are utilizing while attending LCC.
