Wisely Instructions:
  1. Complete the required information on the electronic enrollment form and attach a copy of your government issued document. Wisely by ADP requires a copy of the government issued document on file to help prevent identity fraud. Your Wisely paycard will be immediately issued and available at the Payroll Department, 2nd floor, Washington Court Place building on main campus. Please pick up your Wisely paycard within two (2) business days of submitting the form.
  2. You must provide one of the following items for Paycard identity verification:
    1. Social Security Card
    2. Passport
    3. Driver License number
    4. State ID
    5. Green Card
    6. Visa
    7. Military ID/Government ID
Direct Deposit Authorization

Direct Deposit Authorization Form: Non Employee (New Hire)

Direct Deposit Instructions:
  1. Welcome to the electronic form for Direct Deposit. Complete the required information on the authorization form and attach a copy of your banking documentation. Documentation such as a voided check or letter from your financial institution confirming your routing number and account number must be attached to the electronic form.
  2. Your first (1st) pay will generate a paper check that will be available for pickup on payday Fridays in the Payroll Operations office. Electronic deposits will begin on the second (2nd) pay.
W-4 Federal Tax Withholding
2024 W-4 Federal Tax Withholding Form

The 2024 Federal W-4 contains 5 steps for the employee to review and complete based on personal household income and dependents.
  1. Enter Personal Information
    1. Basic demographic information about employee such as name, address, social security number, and filing status.
  2. Multiple Jobs or Spouse Works
    1. Complete this step if you (1) hold more than one job at a time, or (2) are married filing jointly and your spouse also works.
  3. Claim Dependents
    1. Enter the number of qualifying children under age 17 and/or other dependents.
  4. Other Adjustments
    1. Enter the household information for other income, deductions, and/or extra withholding.
  5. Sign and Date
    1. You are signing this certificate under penalties of perjury.

Steps 1 and 5 must be completed in its entirety on the new 2024 W-4 to be considered a valid and acceptable form. Steps 2, 3, 4 are optional and should only be completed if they apply to you.

The Michigan W-4 contains 9 fields for the employee to review and complete based on personal household income and dependents.
  1. Social Security number (must be all 9 digits)
  2. Date of Birth
  3. Enter Demographic Information
    1. All information must be completed as directed on form.  Include your first name, middle initial, last name, address, city/town, state, and zip.
  4. Driver's License Number or State ID
    1. Must contain all alpha numeric digits on ID.  If you do not have an ID, please write "N/A" in the box.
  5. New Employee – Check Yes or No
    1. Date of hire must be included if you check the Yes box
  6. Enter the number of personal and dependent exemptions you are claiming
  7. Optional Enter the dollar ($) amount of additional taxes you want deducted from each pay
  8. Optional: Only complete box 8 if you can claim exemption from withholding tax.
    1. Check the box a, b, or c based on your "exempt" status reason and complete explanation
    2. DO NOT enter a number in box 6 if you are claiming “exempt” status (leave box 6 blank)
  9. Complete the employee signature and date in box 9
    1. DO NOT complete the Employer section fields (box 10 and 11)

This form must be completed in its entirety as noted above to be considered a valid and acceptable form.

The City of Lansing W-4 contains several fields that an employee must complete based on personal household income and dependents.
  1. Check box "City Resident" or "Non-City Resident"  (You must check one box)
  2. Enter Demographic Information
    1. All information must be completed as directed on form.  Include your social security number, first name, middle name, last name, home address, city, state, and zip code.
  3. Skip to Box 6 - Enter the number of personal and dependent exemptions you are claiming
  4. Box 7 - Optional: Enter the dollar ($) amount of additional taxes you want deducted from each pay
  5. DO NOT complete the Employer's Name and Address
  6. Complete the employee signature and date at the bottom of the form

This form must be completed in its entirety as noted above to be considered a valid and acceptable form.
